The attached checklist needs to be completed and filed with the department for which work is being completed. Please make sure you provide a completed copy to either the Campus Supervisor or Project Manager (in the case of Facilities), or the IT Project Manager or IT professional who you are working with.

KPU Employees:

Steps for KPU Project Managers or those responsible for completing the service request:

  1. Ensure the Contractor has been directed to this link to download and complete the safety checklist
  2. Before the contractor proceeds with the work on site, ensure that you have a copy of the completed checklist
  3. Attach a scanned copy of the completed checklist to the ticket or project for which the work is being done
  4. File a copy of the completed checklist in line with whatever process has been been established by your department (stored in SharePoint or in Department common file folders)
